국소 스테로이드제 선택 기준

• 국소 스테로이드제는 크게 4가지 기전(혈관수축, 항염증, 면역억제, 증식억제)을 통해 작용함.
• 환자의 나이, 도포부위, 병변의 중증도, 순응도 등을 고려하여 선택한다.
• 국소 스테로이제의 세기는 혈관수축 반응 정도에 따라 7단계로 분류된다.
• 같은 역가에 해당되어도 제형에 따라 강도가 달라질 수 있다.

 

국소스테로이드제 역가별 분류

Class 1 (Super potent)

• Betamethasone dipropionate 0.05% ointment (Diprolene and Diprosone ointment)
• Clobetasone propionate 0.05% lotion/spray/shampoo (Clobex lotion/spray/shampoo, OLUX foam)
• Clobetasone propionate 0.05% cream and ointment (Cormax, Temovate)
• Diflorasone diacetate 0.05% ointment (Florone, Psorcon)
• Halobetasol propionate 0.05% cream and ointment (Ultravate)
• Flurandrenolide tape 4 mcg/cm2 (Cordran)

Class 2 (Potent)

• Amcinonide 0.1% ointment (Cyclocort ointment)
• Betamethasone dipropionate 0.05% cream/gel (Diprolene cream, gel, and Diprosone cream)
• Desoximetasone 0.25% cream (Topicort)
• Fluocinonide 0.05% cream, gel, ointment (Lidex)
• Halcinonide 0.1% cream (Halog)

Class 3 (Upper mid-strength)

• Amcinonide 0.1% cream (Cyclocort cream)
• Betamethasone valerate 0.1% ointment (Betnovate/Valisone ointment)
• Diflorasone diacetate 0.05% cream (Psorcon cream)
• Fluticasone propionate 0.005% ointment (Cutivate ointment)
• Mometasone furoate 0.1% ointment (Elocon ointment)
• Triamcinolone acetonide 0.5% cream and ointment (Aristocort)

Class 4 (Mid-strength)

• Betamethasone valerate 0.12% foam (Luxiq)
• Clocortolone pivalate 0.1% cream (Cloderm)

• Fluocinolone acetonide 0.025% ointment (Synalar ointment)

Class 6 (Mild)

• Alclometasone dipropionate 0.05% cream and ointment (Aclovate)

• Desonide 0.05% cream, ointment, gel (DesOwen, Desonate, Tridesilon)
• Desonide 0.05% foam (Verdeso)
• Fluocinonide acetonide 0.01% cream and solution (Synalar)
• Fluocinonide acetonide 0.01% FS oil (Derma-Smoothe)

Class 7 (Least potent)

• Hydrocortisone 0.5%, 1%, 2%, 2.5% cream, lotion, spray, and ointment (various brands)

 

제형별 특징

• 일반적인 강도는 연고, 크림, 겔, 로션, 액 순이다.
• 연고: 건조하고 두꺼운 부위 (다른 제제에 비해 밀봉효과가 커 작용이 증강되나, 제형의 특성상 가려움이나 모낭염이 유발되는 경우가 있음)
• 크림: 급성, 덜 건조한(연고가 필요하지 않은) 병변에 사용, 가장 널리 사용됨.
• 로션: 얼굴에 선호, 넓은 부위에 쉽게 펴 바를 수 있음.
• 액제 및 겔은 두피, 거품 및 스프레이는 털이 있는 부위에 사용됨.
